Advance Directives

Carrington Health Center recognizes that every competent adult has the right and responsibility to make the decisions relating to the adult’s own health care, including the decision to have health care provided, withheld, or withdrawn.

 

Advance Directives include Durable Power of Attorney for Health Care, Living Will and Health Care Directions. Advance Directives provide medical professionals guidelines to the extent of medical treatment you wish to receive. Advanced Directives are for health care issues only, it does not take the place of a regular will for financial matters.”

 

Advance Directives can be filled out at home, your signature needs to be notarized or witnessed by 2 people.

 

Carrington Health Center will gladly send or give you the Advance Directives forms and has Notary Publics on staff.

 

When signing in at the Admissions Office, you will be asked if you have Advance Directives and if you have provided CHC with a copy. If you do not have one, you may receive the packet of information and you may request to have the Social Worker explain the Advance Directives to you.

 

It is important this information remain in your medical record – if you have taken the time to complete an Advance Directive, it is important that your doctor know what decisions you have made.

 

Please provide Carrington Health Center with a copy of your Advance Directives.
